Care navigation and clinical triage are often used as if they mean the same thing. That creates pressure for reception staff and uncertainty for patients. A safer model names three different activities and gives each one an owner.

Where reception adds value
Reception can make the contact safer by confirming identity, using a private communication route, gathering the minimum facts required by the practice process, checking whether a request is already open and explaining who owns the next step. That is skilled work. It does not require a receptionist to decide what condition the person has.
Where the boundary is crossed
The boundary is crossed when someone interprets symptoms, creates their own exclusion criteria, downgrades because the day is busy or reassures a patient that a problem is not serious. The correct response to an unclear pathway is not a best guess: it is the named senior or clinical support route.
Capacity does not change need
A full appointment book may change the practice's overflow or escalation process, but it must not silently change the route assigned by the approved model. Training should rehearse this pressure explicitly, because it is when good processes are most likely to fail.
Language that keeps the boundary clear
Useful wording describes the process: “I will record what you have told me and send it to the team that assesses these requests.” Avoid promises such as “the pharmacist will give you antibiotics” or clinical reassurance such as “it sounds routine”.
